Documents in your child’s old name

We need to know about any current records which are still in your child’s former name.

For documents / records (potentially) still in your child’s former name, bear in mind that what counts is the name that organisations currently know your child by — which may be different from documents or letters that were issued in the past.

The name an organisation currently knows your child by will be —

• the name on your child’s last issued passport
• the name on the school register, and on school reports
• for medical records — the name your GP calls your child by when you visit your local surgery, and on any prescriptions
• for Child Benefit records — the name on letters / official forms you get from the Child Benefit Office
