How your name will be displayed on the deed poll

Normally Deed Poll Office will show all surnames in capital letters on a deed poll, to distinguish them from the forenames. The forenames are shown with the first letter of each word capitalised.

So for example, the name John Michael Smith would be displayed as:

John Michael SMITH
